Thanks for this. Your production incident captures the real inflection point. Most teams chase accuracy because it feels quantifiable. What they miss is that the 8 percent explicit “I do not know” is the only thing that keeps an agent inside the boundary of what the organisation can legally defend.

Precision without the ability to refuse is not intelligence. It is liability at scale.
